I recently upgraded from a 1080p to a 4K monitor. I now use the default 4K resolution (3840x2160). I use Preset 5 from the preset options (2 large thumbnails and 16 smaller ones). In "Layout", I had set "Fit Screen" and all previous "Preset 5" images output to 1191x1080 in image size. This was fine on my old 1080p monitor because when viewing the thumbnail sheet, it would open small but then a click on my default image viewer program and it was full size on screen. However on my new 4K monitor, the same settings output the image to 1615x1434. So it is like 40% bigger than the previously made thumbnail sheet on the previous monitor, but it is not "Fit Screen" like it used to be. In fact it is no where near fitting the screen. When viewing this new sheet on the 4K monitor, even at 100% image size it is only occupying 2/3rds of the screen at full view (1434 pixels of 2160 pixels). I want the image to be from the top to the bottom of the monitor like it did with my old 1080p monitor. The new image/Preset 5 keeping the same settings should be outputting to 2400x2160 more or less. I don't like to use "As Original" because the dimensions are just too big, both in resolution and file size. I much prefer something that is 100% size based on the monitor resolution. Hopefully you can look into this and sort it out because I needed to re-do all my old thumbnail sheets (1000's!) due to the new higher resolution, so when this didn't do the full size, it became an issue. Thanks!

It looks like your screen resolution is not 4K, but 2K (2560x1440).
Could you please check that in Windows settings? Please provide a screenshot.
Also I would like to check your Options file (you can export it).

I 100% know that my monitor is running at a 4K resolution. I know this because it is one of the main reasons I upgraded and was probably the first thing I setup when I connected it. I have included both screenshots from within the Display properties and from my monitor OSD to show that the monitor is 100% at a 4K resolution. I will also attach the options file for you to have a look at.

Thank you.
Ok, it is related to the Windows scaling on your first screen.
Could you please try to set Windows scaling back from 150% to 100% and try again?
It should be ok.

1) I changed the Windows scaling to 100% and when I did this, the output file was the correct size. It had an image resolution of 2471x2160. So it seems the scaling is the factor in all this. Please note that I have to have scaling on (150%) because otherwise text is too small to read. So as much as I would like to stay at 100% scaling so I can get the correct size thumbnails, it is just not an option. I hope you can figure out a way to get the app to ignore the scaling factor when outputting via "Fit Screen".

2) Yes my second monitor is a 1080p monitor (not the old one). This dual monitor setup in only 2 weeks old and consist of 1 32" monitor running at 3840x2160 and a 2nd 24" monitor rotated vertically running at 1080x1920. My previous setup was 1 32" monitor running at 1080p and a 2nd 20" monitor running at 1080p. I have always had a dual monitor setup since the very beginning of VTM. There has never been any issues in VTM due to a dual setup as far as I am aware. I don't run VTM on the secondary monitor, only the main one.

Since you edited what you originally wrote, let me reply again. Yes, scaling back to 100% fixes the issue, but that is NOT the long term fix. The solution cannot be "put your system back to 100%..." The scaling to 150% is necessary because text is very hard to read at 100% on a 4K resolution monitor. I'm not putting the scaling to 150% because I want to, I'm doing it because I have to. I can't be expected to put scaling back to 100% every time I need to run VTM just so it will make the correct size jpg. I run VTM randomly many times throughout the day so this would be a big inconvenience. This will only get worse in the long run as monitors move to 4K, given that 8K monitors already exist. Obviously it won't be a problem for a while but a proper long term solution to handle high resolution monitors and their scaling needs to be put in place now so it doesn't become a bigger problem in the future. Hopefully you will either find a way for VTM to bypass/ignore the Windows scaling but respect the resolution OR allow us to manually set the size of the output image for presets. Either solution is fine with me. Thanks for your consideration!
